chore: track .ingest-manifest.yaml used by /gsd-ingest-docs

Preserves the explicit-manifest input used to bootstrap .planning/ so the
ingest is reproducible. Two docs were classified: Тз расширение фаза1.md
(SPEC, precedence 0) and README.md (DOC, precedence 1).
