docs(state): record phase 3 UI-SPEC session — null-spec approved 6/6 dimensions

+ percent:100 since 18/18 currently-known plans are done. Restored:
status:ready_to_plan, percent:50 (2/4 phases truly complete).

auto-deletes when consumed).
